Canadian Forces Case Management

The Case Management Program is designed to help eligible CF members identify and access health, pension and other related resources and to ensure continuity of care.

The CF Case Management Program has more than forty Case Managers across the country to provide you with the best care possible. A Case Manager is available at most CF Medical Clinics. For referral or contact information please contact your Base Medical Clinic.

CF members who have a medical condition or illness and requires case management services.
